Witness Statement
Sole Responsibility Witness Statement 模板
WITNESS STATEMENT OF [MOTHER’S FULL NAME]
I, [Mother’s Full Name], of [Current Address], make this witness statement in support of my child’s UK visa application.
I confirm that the contents of this statement are true to the best of my knowledge and belief.
1. Background
I am the biological mother of:
Child’s Full Name: ____________________
Date of Birth: ____________________
I currently reside in the United Kingdom under a valid visa/immigration status.
The child’s father is:
Father’s Full Name: ____________________
We divorced on/about ____________________.
Following the divorce/separation, our child has remained under my sole and primary responsibility.
2. Family Circumstances
Following the breakdown of our relationship, I became the primary parent responsible for all major aspects of my child’s upbringing, welfare, education, healthcare, and daily arrangements.
Although my child has been physically residing in China during part/all of this period, I have continued to exercise sole and primary responsibility for all important decisions relating to my child.
Any assistance provided by grandparents or relatives has been limited to day-to-day practical support only and not decision-making responsibility.
3. Educational Responsibility
I have been solely responsible for my child’s educational arrangements, including:
selecting schools and tutoring institutions;
communicating with teachers and school staff;
arranging classes and educational activities;
attending or arranging parent meetings where possible;
paying tuition fees and educational expenses; and
making all important educational decisions.
The child’s father has not played an active role in the child’s education.
Supporting evidence includes:
school letters;
tuition payment records;
communication records; and
tutoring institution letters.
4. Medical Responsibility
I have also been solely responsible for my child’s healthcare and medical arrangements, including:
arranging medical appointments;
communicating with doctors and hospitals;
making medical decisions;
paying medical expenses; and
managing the child’s overall wellbeing.
The child’s father has not been substantively involved in the child’s healthcare arrangements.
Supporting evidence includes:
medical letters;
hospital records; and
healthcare payment records.
5. Financial Responsibility
I have continuously and financially supported my child throughout the relevant period.
I regularly pay for:
school fees;
living expenses;
medical expenses;
tutoring fees; and
other daily needs.
Supporting evidence includes:
bank statements;
transfer records; and
payment receipts.
6. Relationship with the Child’s Father
The child’s father has not exercised primary responsibility for the child’s upbringing.
He has not been actively involved in major decisions relating to:
education;
healthcare;
welfare; or
day-to-day management of the child’s life.
[Optional paragraph if applicable:]
The child’s father has provided his consent for the child’s UK visa application and relocation to the United Kingdom.
7. Child’s Current Care Arrangements
As I currently reside/work in the United Kingdom, my child has temporarily been staying with grandparents/relatives in China.
However, they only assist with practical day-to-day care.
All important decisions concerning the child continue to be made by me directly.
I remain in regular daily/weekly contact with my child through phone calls, video calls, and messaging applications.
8. Reason for the Application
I wish for my child to join me in the United Kingdom so that we may live together as a family and so that I can continue directly caring for and supporting my child.
It is in my child’s best interests to reside with me in the United Kingdom.
9. Conclusion
I respectfully request that the Home Office consider the evidence provided and accept that I have sole and primary responsibility for my child.
I confirm again that the contents of this witness statement are true.

“The father has not exercised primary responsibility.”
6. Witness Statement
Witness Statement
+
1. Statement
